By accessing or using the Service you agree to be bound by these Terms. If you disagree with any part of the terms then you may not access the Service.

Purchases

If you wish to purchase any product or service made available through the Service (“Purchase”), you may be asked to supply certain information relevant to your Purchase.

Participation Waiver:

Participation in all sports and physical activities involves certain inherent risks and regardless of the care taken, it is impossible to ensure the safety of the participant. A variety of injuries may occur to a participant. Some examples include, but are not limited to: (1) Minor injuries such as scrapes, bruises, strains, and sprains; (2) Serious injuries such as fractures, dislocations, tom ligaments, tendons, and muscles, concussions, and heat illness; (3) Catastrophic injuries such as paralysis and death. Athletes have a responsibility to prepare properly and participate in a safe manner to enhance the safety and enjoyment of the sport for themselves and others. By checking the checkbox in the checkout of this website I acknowledge that I understand the inherent risk of sport and agree that my child may participate in Ultimate.

Media Agreement:

Elevate Ultimate Academy is eager and proud to showcase our participants, coaches, and staff and/or the work that they have done, however, we have a responsibility to protect the privacy of our participants and staff. In accordance with the Freedom of Information and Protection of Privacy Act, Elevate Ultimate Academy requires parental consent for the use of a participants’ image or work produced. Elevate Ultimate Academy requests your permission to take/use or have media take/use photographs/audio visual recordings of your child participating in academy activities. These photographs/audio visuals might then be used for external public viewing, on the academy website, in printed promotional materials and/or used by news media (print/electronic). By checking the checkbox in the checkout of this website I acknowledge that I understand and give my consent for my child’s image and/or work they produced to appear in photos/audio visual recordings while participating in an academy activity and that these may be used for promotional purposes on externally distributed printed materials, on our website or broadcasted by electronic media.
